Interventions for Sexual Issues and Dysfunctions LaShawnda Ogle Walden University The Wilson’s are currently experiencing some sexual dysfunctions, a condition that can cause a lot of distress during intercourse (Hecker & Wetchler, 2010). In the case study Mrs. Wilson has been diagnosed with having Vaginismus. Vaginismus is known for causing some severe pain and as a result causes the female to avoid sexual activity (p. 377). It’s formally defined as the involuntary spastic contraction of the outer one-third of the vagina. There can be treatment techniques. The two sexual therapy interventions I would suggest for the Wilson’s are Cognitive Behavioral Therapy (CBT) and a medically treated approach using a vaginal dilator. The one intervention method suggested for Mrs. Wilson would be that of Cognitive Behavioral Therapy. According to Hecker & Wetchler, (2010), women with vaginismus can show fear and have some anxiety with the thought of penetration. In order to help decrease those fears and anxieties CBT may help Mrs. Wilson’s sexual satisfaction increase as well as her over all well-being: addressing not only the physical pain or discomfort of sex but also the psychological aspects. For example: there are cognitive strategies such as: sexual exercises and relaxation techniques that can be applied with CBT in order to help relieve the pain associated with vaginismus (Hecker & Wetchler, 2010). CBT can also offer some coping skills along with restructuring a person’s way of thinking. Another intervention treatment method for sexual disorders such as that of Mrs. Wilson is more of a medically treated approach. Hecker & Wetchler, (2010) suggest a vaginal dilator as being a useful technique for modifying a conditioned response. Vaginal dilators are used to teach control of circumvaginal muscles (p. 377). In order to use these dilators they are generally supplied to people from a doctor’s office with or without the presence of the person’s partner. The private space offers the opportunity to help decrease the fear and anxiety with sex so that penetration can occur. The similarities of both CBT therapy and the medically treated approach using a vaginal dilator are that they both focus on calming the painful symptoms associated with sexual discourse in females. Another similarity with the two interventions is that they both offer suggestions in how to cope and deal with the shame and/or guilt associated with sexual dysfunctions. The only differences are the methods used to intervene. CBT focuses more on the cognition of dealing with negative thoughts that often occur with sexual disorders. The vaginal dilators focus on ways to relieve the stress without the conditioned fear response (p. 377). A challenge with using CBT therapy and vaginal dilators is that it encourages having both partners actively involved (Hecker & Wetchler, 2010). Sexual intercourse and dysfunctions can be a touchy subject that does not resolve itself without partner interaction, so getting both partners together could be somewhat of a challenge. In order to know whether or not a treatment intervention is working, both Mr. and Mrs. Wilson need to report the improvements in the symptoms of Mrs. Wilson. I would also like to know from both partners if increased enjoyment has occurred after treatment, in order to know whether or not my treatment interventions were working. Howard Zinn and the Us Constitution

The late Howard Zinn is a much respected historian. His views are known to be bold and nonetheless controversial. In his book, â€Å"A People’s History of the United States,† Zinn touches on topics such as indentured servants, angry civilians, and the United States Constitution. Indentured servants were people of a lower economic class who worked for people of a higher economic background. These servants worked for a given amount of time, usually between five and seven years and either worked for money, food, shelter, or freedom.Indentured servants were originally made up of mostly young white males who were trading their time in prison or their poverty for time working as a servant. The number of indentured servants began to decrease and soon after English colonists looked for other potential people to enslave. The Virginia colony needed labor. They needed to grow corn for subsistence, and needed to grow tobacco for export because they had just learned to grow tobacco. Virginia couldn’t make the Indians work for them like Christopher Columbus had done in the past. The colonists would be outnumbered if they decided to try to take over the Indians even though they were equipped with firearms. The Indians were resourceful, defiant, tough, and practically fearless. The colony needed an alternate choice. African slaves were the answer to Virginia’s labor problem. Blacks had already been imported as slaves to South America and the Caribbean to Spanish and Portuguese colonies.The blacks made enslavement easier because of how hopeless they were. They were robbed of their homeland and culture and in most cases they were separated from their families. Zinn referred to the slavery against the blacks to be the cruelest form of slavery in history. The British were taxing the colonial population to pay for the French war. Many colonists did not agree with the Stamp Act and wanted it repealed.That summer, Ebenezer Macintosh, a shoemaker, led a mob in destroying the house of a rich Boston merchants like Andrew Oliver and Thomas Hutchinson. Rioters smashed up their houses with axes, drank all the wine in the cellars, and looted the houses of the furniture and other objects. English officers reported these acts to be a part of a larger scheme in which the houses of 15 rich people were to be destroyed. The riots against the Stamp Act swept Boston in 1767.It took the Stamp Act crisis to make the leadership aware of its dilemma. After the riots a town meeting was arranged and mainly upper and middle class citizens were allowed to attend. Zinn argues the Founding Fathers, Benjamin Franklin, George Washington, John Adams, Thomas Jefferson, John Jay, James Madison, and Alexander Hamilton, may have had ulterior economic and class preservation motivations that were hidden by the universal language of the constitution document.Zinn also argues that the rich, in order to secure their own interests and economic status, must either control the government directly or control the laws by which government operates. Zinn often refers to the views and writings of historian Charles Beard. Beard studied the economic backgrounds and political ideas of the fifty-five men who gathered in Philadelphia in 1787 to draw up the constitution. In his findings a majority of them were lawyers by profession, most of them were wealthy due to land, slaves, manufacturing, or shipping.Half of them had money loaned out at interest, and that forty out of fifty held government bonds according to the records of the treasury department. Beard also found that most of the makers of the constitution had some direct economic interest in establishing a strong federal government. Beard did not think the constitution as written to benefit the Founding Fathers personally. The problem of democracy in the post- revolutionary society was not however the constitutional limitations on voting.It lay much deeper beyond the constitution in the division of societ y into rich and poor. The constitution then illustrates the complexity of the American system: that it serves the interests of wealthy elite, but also does enough for small owners, for middle-income farmers and mechanics to build a broad base of support. Alexander Hamilton, Secretary of Treasury and one of the Founding Fathers, believed that the government must ally itself with the richest elements of society to make itself strong.

When an individual becomes a resident of a certain community that makes use of the target language in its daily interactions, integrative motivation becomes the key element in developing the level of language proficiency. It becomes mandatory for the individual to function socially within the community and becoming one of its members (Norris-Holt, 2001). On the other hand, instrumental motivation is characterized by the need to gain something concrete or practical from the learning of a second language. The goal of acquiring second language in instrumental motivation is utilitarian, for instance, meeting the necessities for university or school graduation, application for a job, reading technical material, attaining higher social status, translation work, or request for high pay based on the language ability. Instrumental motivation is common in in stances where the acquisition of a second language is not important for the learner’s social integration into the society (Norris-Holt, 2001). Both instrumental and integrative motivations are important elements for success but it has been found that integrative motivation sustains long-term success in the learning of a second language. Research shows that integrative motivation is important in the formal learning setting or environment. It is important to note that both instrumental and integrative motivations are not necessarily mutually limited. Learners rarely choose one type of motivation when studying a second language. Instead, the learners combine both orientations. For instance, international students living in the United States learn English for academic reasons and at the same time, they desire to become incorporated with the culture and the people of the country. Motivation is an essential factor in second language achievement. Thus, it is crucial to determine the combination and form of motivation that helps in the effective acquisition of a foreign language (Norris-Holt, 2001). Communication Paper Essay I would like to take this time in welcoming you to this presentation on communication marketing in health care. We will first be identifying different and most common communication used by consumers and health care providers, such as e-mail, a web-based forum, and/or electronic medical records just to name a few. We will also be discussing the benefits to the patients, an aspect relating to the values and importance of maintaining patient confidentiality when using these types of communication, some reasons that these types of communications are an effective way of communicating between consumers and their providers, how these types of communications may differ from others, and lastly, how might media and social networking change communication in health care today and years to come. Now, join with me as we discuss the different types and most common communication used by consumers and their health care providers. Many communication channels are used in healthcare, each with different properties. These channels may be synchronous or asynchronous, mobile or fixed, secure or no secure. Because of this, participants must decide which channel best suits the issue about which they need to communicate. Synchronous channels such as telephones and face-†to-†face meetings allow both parties to communicate at the same time, enabling information to flow freely in real time. Synchronous channels are ideal when discussing time-†sensitive issues and exchanging dense information. It is not always convenient, however, for a patient and physician to schedule time to communicate using synchronous channels because they can lead to delayed care, telephone tag, and frustration—when these channels fail to connect, some healthcare issues remain unaddressed. Yet, generally only pagers and synchronous communications are available for patient-†physician  communication. Asynchronous channels such as email, fax, and blogs enable each party to communicate when it is convenient. These channels may be used safely for issues that are no urgent or time-†sensitive. Because time is not a factor when using asynchronous channels, both patient and clinician can be more reflective in their messaging. Asynchronous channels, however, should not be used for medical emergencies or time-†sensitive issues. Another characteristic of communication channels is whether they are fixed (connected to a wire) or mobile. Thanks to advances in technology, many communication channels that were once fixed are now wireless, such as telephones and text messaging. Presenting new technologies is challenging when dealing with an industry entrenched in using the same telephone, fax, and pager systems for decades. Many problems in healthcare stem from the industry clinging to these outdated methods of communication, without understanding the benefits brought about by newer solutions. For example, electronic pagers, which have been abandoned by most industries, are still the core device for hospital communications. While physicians extensively use a mobile core phone outside the hospital, these phones are often banned from being used inside, despite evidence that in most areas of the hospital this technology is safe. Email is often discouraged for clinical communication for security reasons, and more advanced technologies such as instant messaging, videoconferencing, group teleconferencing, or web-†based collaboration are almost unheard of in clinical care—despite evidence that proves the effectiveness and safety of these technologies in healthcare. While physicians have found newer tools such as email useful, they are typically used only in limited situations and not commonly employed to communicate with patients. The reasons for physicians’ reluctance to use e-†communication with patients include concerns about malpractice litigation, fears of being barraged with emails, and the absence of a model that reimburses them for consulting with patients electronically. Communication in healthcare fails for many reasons: necessary personnel cannot be identified or located; they may not be available or may not respond in a timely manner; or the communication channel of choice may not be well suited to the task at hand— for example, e-†communication such as  email should not be used for time-†sensitive issues such as a patient having a heart attack, or in situations that require a dense exchange of information. Pagers, for instance, are more appropriate for time-†sensitive issues, while telephones, in-†person or videoconferencing meetings, or web-†based collaboration tools are more efficient options for exchanging dense information, such as detailed c ase reports. Because case reports require much discussion among physicians, using email only slows down the discussion process. With the ability to reduce costs, telemedicine has been shown to be such an effective medical practice in several instances that its growth and application in the health care industry have raised greatly. One of the reasons for patient satisfaction with telemedicine is a reduction in waiting time, travel time, and the time involved in arranging appointments. The absence of all these issues can facilitate health communication by eliminating many of the burdens involving in standard health care. There also some challenges to the patient privacy with web-based communication just as in a facility. Because multiple individuals in telemedicine communication, exposure of confidential records to all parties concerned becomes a threat to the privacy of the patient. Additionally, even through medical doctors accept the obligation of maintaining their patients’ privacy rights, the other assisting parties involved in the telemedicine communication may not be held to the same standards (Turner, 2003). As a result of this risk to patients’ privacy rights, telemedicine has struggled to gain acceptance for the legal and medical communities (Sanders Bashshur, 1995). Examples like these can be shown all over history books, in stories, tales and legends across the entire world. Women have been subordinate to men in virtually all societies throughout history.   Ã‚  Ã‚  Ã‚  Ã‚  The ideology that one sex is superior to the other is called sexism. The presumption of male sexism led to patterns of prejudice and discrimination against women. These prejudices and discriminations have led to many beliefs or ideas of why women are inferior to men. They range from brain size to sexual differences, including personalities based on genitalia. Cross-cultural studies demonstrate how the socialization process and societal expectations of men and women produce variances in sex-role norms and behavior. As the realization of women as an exploited group increases, the similarity of their position to that of racial and ethnic groups becomes more apparent. Women are born into their sexual identity and are easily distinguished by physical and cultural characteristics. In addition, women now identify that they are all sufferers of an ideology (sexism) that tries to justify their inferior treatment. In all societies around the world, women are treated as if they are a minority group, just like any racial or ethnic group that is out of the norm. The justification for considering women as a minority group and the existence of sexism becomes clear through the examination of social indicators, including education, employment, and income.   Ã‚  Ã‚  Ã‚  Ã‚  Education was sex segregated for hundreds of years. Men and women went to different schools or were physically and academically separated into â€Å"coeducational† schools. If women were paid equal to men, more families may be able to pay for child care expenses and working mothers could continue to work and get ahead. Women have been treated unequally since the beginning of time. Just recently have things began to change for the better for women and the future of our society. The increase in women’s equality rights will take time, but some day women and men will be treated equally. This cannot happen until each of us is able to look at a person and just see another individual, not a male or a female, white or black, rich or poor†¦ a person as just a person.

I would like to present myself as a prospective candidate for Master’s Program in Mechanical Engineering at Masdar Institute of science and technology. I graduated from NED University of Engineering and Technology as a Mechanical Engineer in December 2011. The reason I opted mechanical engineering was my inclination towards the subjects relating mechanics of solids. My interest in this field grew to such an extent that I became passionate of taking the knowledge gained in the classrooms to the next level and was successful in securing the final year project of â€Å"Designing of Pipeline for Refinery Effluent†, a real time industrial assignment of Pakistan Refinery Limited which is considered as one of the leading oil and gas companies in Pakistan. To secure such an industrial project, which not only needed research potential but also the aspect of understanding the dynamics involved in the industry, I prepared myself during my bachelors by participating in numerous projects and received multiple national awards for my work, as a leader and a member of team for the design and fabrication of unmanned aerial vehicle (UAV) in DBFC (Design Build and Fly) organized by the American Institute of Aeronautics and Astronautics (AIAA). I was also selected for the renowned international competition Shell Eco Marathon Asia, to design an energy efficient car. These projects not only gave me hands-on experience in dealing with composites and testing my knowledge, they also gave me the chance to explore the field of fluid mechanics and aerodynamics. Along with these notable competitions, I regularly took part in local engineering competitions and seminars to actively apply the knowledge I gained during my bachelors.
